Under immediate supervisor, has general responsibility for supervising and coordinating the daily activities of home care aides. Primary responsibilities include scheduling client services, filling needed staff call-outs, orientation, obtaining client authorization, and maintaining regular contact with clients.
Essential Duties and Responsibilities- Manage client caseload and service delivery.
- Assist with coordinating client and home care aide schedules.
- Assign home care aides to new and current clients based on client needs.
- Coordinate emergency fill-in care when needed.
- Provide care for clients on an emergency fill-in basis as necessary.
- Maintain a standby name list for available substitute home care aides.
- Assist with fill-in shift needs as directed.
- Consult with HC Team regarding staffing needs and communicate to recruiter.
- Orient caregivers to client plans of care and document.
- Assist with day‑to‑day operational needs of HC Department as directed by HC assistant director and director.
- Maintain current employee and client schedules.
- Conduct new client intakes and thoroughly assess care needs.
- Periodic supervisory visits as well as meet and greets.
- Supervise home care aides.
- Complete home‑care aide evaluations according to agency policy.
- Respond and resolve home‑care complaints and issues.
- Consult with HC Program Manager regarding employee disciplinary actions when necessary.
- Complete Supervisor report weekly.
- Report all unusual occurrences regarding client case to HC Program Manager.
- Correspond and communicate as necessary with Case Management and clients.
- Document all communication in Therap daily.
- Conduct home visits and telephone contacts to clients according to agency policies and procedures.
- Enter new client information into Therap within one working day.
- Keep clients informed weekly regarding status of client care coordination.
- Enter all client supervisory information.
- Ability to read and comprehend instructions, short correspondence, memos, business correspondence and all other mental‑health related documents.
- Ability to write with clarity, conciseness, and accuracy.
- Proficiency in Microsoft Office (Word, Excel) and internet.
- Excellent documentation and mathematical skills.
- Establish positive interpersonal relationships with a broad range of people.
- Effective oral and written communication skills.
- Maintain confidentiality.
- Good organizational skills.
- Resolve complex problems and handle instructions furnished in written, oral or diagram form.
- Work with minimal supervision; function autonomously and make individual decisions when appropriate.
- Possession of a valid Washington driver’s license is required.
- Pass criminal history check.
- Experience in related field.
- Ability to train, develop, and support new staff members.
- Two years of supervisory experience preferred.
- Prior experience in providing care to the elderly and/or disabled; all aspects of personal care including transfers.
- CNA or Home Care Aide Certification preferred.
- Work in an office environment.
- Travel to clients’ residences for home visits and direct care.
Type: sedentary work with administrative duties, typing, filing, bending over to low files, reaching high files. Work involves sitting most of the time, but may involve walking, standing and driving for extended periods. Must be physically and mentally capable of caring for an individual who is ill, infirm, disabled or vulnerable.
