Job Description
Jethro Health System is looking for a Clinical Nurse Manager or Director of Nursing to join our team. The Director of Nursing/Clinical Nurse Manager is an RN who provides oversight of all patient care services and personnel, ensuring quality of patient care in the home. The role includes coordination with the interdisciplinary team, patients, families, and referring agencies. It may also be titled Director of Nurses, Supervising Nurse, Director of Clinical Services, Director of Professional Services, or a similar title.
Requirements- Education: Graduate from an accredited college
- Licensure: Active state license as a registered nurse (RN).
- Experience: 3 years of experience in home health care preferred. Management and leadership experience preferred.
- Skills: Clinical skills as defined by generally accepted standards of practice, excellent interpersonal and communication skills, proven decision‑making, and proof of current CPR certification if required to make visits.

Working Conditions:
Performs duties in an office environment while also making patient visits as required. Possibility of exposure to blood, bodily fluids, infectious diseases, unpleasant weather, and PRN emergency calls. Flexible schedule and local travel required.
The Clinical Manager supervises and coordinates clinical staff to ensure adherence to CMS Conditions of Participation, state regulations, and agency policies
. Responsibilities include overseeing patient care services, quality assurance, staff training, compliance with physician orders, and maintaining high standards of patient care, staff development, and operational efficiency.
Clinical Supervision & Patient Care Oversight
- Ensure timely, high‑quality, and compliant delivery of home health services.
- Supervise nurses, therapists, and other clinical staff, providing guidance and support.
- Review patient assessments, care plans, and physician orders for accuracy and completeness.
- Ensure staff compliance with OASIS documentation, PDGM requirements, and agency protocols.
- Conduct case conferences to review patient progress and update care plans as needed.
Quality Assurance & Compliance
- Implement and oversee the Quality Assurance Performance Improvement (QAPI) program.
- Conduct chart audits to ensure proper documentation and regulatory compliance.
- Monitor patient outcomes, hospitalizations, and infection control data to identify areas for improvement.
- Ensure compliance with Medicare, Medicaid, and state home health regulations.
Staff Development & Training
- Provide clinical education, mentoring, and performance evaluations for staff.
- Conduct orientation and ongoing training on documentation, compliance, and best practices.
- Address staff performance issues, ensuring adherence to clinical policies and procedures.
Operational & Administrative Responsibilities
- Coordinate patient admissions, discharges, and staff assignments to maintain workflow efficiency.
- Collaborate with physicians, referral sources, and case managers to ensure continuity of care.
- Assist in the development and revision of clinical policies and procedures.
- Participate in state surveys, accreditation reviews, and compliance audits.
- Oasis knowledge, including hands‑on experience with review.
- Coordinate patient care and ensure communication with all physicians involved in a patient’s plan of care and other healthcare practitioners.
- Integrate orders from all relevant physicians into the plan of care, ensuring approval by the responsible physician.
- Coordinate and integrate care provided by agency staff, including contractual providers.
- Coordinate referrals.
- Assure that patient needs are continually assessed and reassessed when significant changes occur.
- Develop, implement, and update individualized plans of care with patients, representatives, and caregivers.
- Ensure provision of services as ordered by the physician, including patient and caregiver education and counseling.
