Experience as a Home Health Agency RN required to care for patients in the North Shore area:
Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, Revere.

This is a part‑time position with the potential to become full‑time.

Responsibilities
  • Conduct thorough assessments of patients’ health status and needs in their home environment.
  • Develop and implement individualized care plans based on patient assessments and medical history.
  • Provide education and support to patients and their families regarding health conditions, medications, and self‑care techniques.
  • Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.
  • Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.
  • Document all patient interactions accurately in compliance with ICD‑10 coding standards.
  • Offer health coaching to empower patients in managing chronic conditions effectively.
  • Provide specialized services such as esthetic laser treatment when applicable.
  • Support family planning initiatives and educate patients on related health topics.
Skills
  • Strong clinical skills with a focus on acute care and senior care practices.
  • Excellent communication skills for effective patient education and collaboration with healthcare teams.
  • Proficient in discharge planning processes and the ability to navigate complex healthcare systems.
  • Knowledge of ICD‑10 coding for accurate documentation of patient encounters.
  • Experience working in a teaching hospital environment is preferred but not required.
  • Ability to provide compassionate care while maintaining professional boundaries.
  • Strong organizational skills to manage multiple patient cases effectively.
  • Commitment to ongoing professional development and staying current with best practices in nursing care.

Pay: $38.00 – $45.00 per visit
