RN Case Manager – Recuperative Care Center – Weekend Hours

Lynn Community Health Center is seeking a talented and compassionate RN Case Manager to support respite patients at the Deborah Walnut Recuperative Care Center.

Base pay: $35.50/hr – $50.00/hr actual pay will be based on your skills and experience.

• Provide direct nursing care to ensure all patient needs are met, conforming to established policies, procedures, and Public Health regulations.
• Support engagement of patients with substance use disorder in a manner consistent with Harm Reduction Principles, provide risk reduction counseling and connect to treatment programs.
• Communicate pertinent abnormal test results to provider.
• Knowledge of opioid overdose management and brief interventions.
• Have interest in working with underserved populations.
• Utilize those skills to interact with patients/clients, team members and staff from other departments.

• Graduate of an accredited nursing program, current Mass. Licensure, BLS certification.
• Bi-lingual in Spanish /Khmer/Russian with knowledge of medical terminology preferred.
• Previous Community Health or Ambulatory Experience preferred.
• Understanding of substance abuse and mental health issues.
• A desire to work as a collaborative member of a multi-disciplinary team.
