LPN, Healthcare Nursing, Private Duty Nurse

• Supports the Registered Nurse in providing care based on each patient’s plan. Assists with procedures as directed by the RN and physician, helps carry out and review care plans, and provides emotional and physical support to patients and their families. Educates patients and families on proper self-care, attends team meetings, and completes visit notes after each patient interaction. Monitors and reports any changes in a patient’s condition, takes part in a rotating weekend on-call schedule, and completes other duties as needed.

• Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of home health experience preferred
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.
