The Acute Care Clinical Social worker provides social work services to patients and their families, maximizing utilization of personal, family, community, and health care resources to formulate an appropriate safe discharge plan in the acute care setting.

Responsibilities
  • Works with case management to understand the patient’s utilization plan.
  • Evaluates or assists the attending physician and interdisciplinary team in assessing the psychosocial needs of the patient.
  • Works collaboratively with the case manager and interdisciplinary team to develop and implement a comprehensive integrated discharge plan in the acute care setting.
  • Completes initial and ongoing assessment of the patient and family response.
  • Assesses the capacity of the home caregiver to cope with the patient’s needs while developing the plan of care.
  • Completes appropriate psychosocial assessment and referrals.
  • Identifies patients requiring crisis intervention and acts promptly to resolve issues and prevent barriers to patient flow.
  • Conducts a comprehensive patient/family assessment.
  • Assesses the patient’s third‑party payer coverage to facilitate development of a post‑acute discharge plan.
  • Clearly documents the discharge plan and updates in the medical record on an ongoing basis.
  • Provides ongoing education to patients and families regarding care planning and discharge preparation.
  • Maintains systems to accurately track and trend variances in care and service.
  • Acts as a leader supporting psychosocial treatment and outcome goals.
  • Serves as a role model, sharing knowledge and practice standards through participation in a formal social work internship program as a preceptor.
  • Follows advanced learning program guidelines and provides feedback for social worker intern development.
  • Serves as departmental preceptor for new social work staff, promoting quality care and evaluating performance for improvement.
