Responsibilities
  • Serve as RN Case Manager in a long‑term care setting, assessing, implementing, and coordinating care plans for residents.
  • Provide wound care and obtain wound‑care certification within 1 year of hire.
  • Participate in QAPI activities and restorative practices.
  • Perform rotational duties: one medical‑director rotation per month and one charge‑nurse shift per week.
  • Administer medications, ensure accuracy of the MDS, and process physician orders.
  • Educate residents, families, and the care team, facilitating communication and documentation.
  • Monitor and evaluate resident outcomes, maintain fiscal responsibility, and manage variances.
  • Work primarily day hours with scheduled weekends and occasional evenings, with potential exposure to communicable diseases, hazardous materials, and injury risk.
Job Summary

The RN Case Manager, Long Term Care (LTC) assesses, implements, and coordinates a plan of care for residents in pertinent areas. This position ensures medications and treatments are administered to assigned residents, assists with plan of care and interventions for effectiveness, and assesses the accuracy of the MDS. The role focuses on providing residents with opportunities for independence while maintaining self‑esteem, privacy, and confidentiality.

Physician orders are processed and physicians are notified in a timely manner. Care is documented and residents, families, and the care team are educated. Outcomes are evaluated by integrating clinical, financial, and utilization data. The RN works in both structured and unstructured settings, uses independent nursing judgment, and applies age‑specific structured care methodologies such as protocols and integrated pathways. The RN interviews potential residents, assesses compliance with regulations, uses assessment information for admission and continued stay, and ensures all residents meet assisted‑living qualifications.

Service plans are established and evaluated on an ongoing basis. The RN may work with all age groups and must communicate effectively, facilitate groups, and implement projects as needed. The role requires independence, assertiveness, critical thinking, clear documentation, problem‑solving, and self‑motivation with strong flexibility and organizational skills. Knowledge of reimbursement for services provided is essential. The RN works primarily day hours, scheduled weekends, and occasional evenings, with potential exposure to communicable diseases, hazardous materials, injury from assigned duties, and multiple sensory and environmental stressors.

Qualifications
  • Bachelor’s degree in Nursing strongly preferred.
  • Graduation from an accredited nursing program (CCNE, ACEN, or NLN CNEA).
  • Minimum of two years of clinical experience preferred.
  • Current unencumbered RN license with the State Board of Nursing in the state of practice; multistate licensure acceptable for NLC states.
  • Ability to obtain and maintain required department‑specific competencies and certifications.
