Position: RN Case Manager - Full Time Days - LTC

Position Title

Full Time Days – RN Case Manager (Long Term Care)

Compensation and Benefits
  • $40.00 - $51.00 per hour (salary range)
  • Eligible for medical, dental, and vision benefits
  • Additional benefits include: wound care certification within 1 year of hire, QAPI participation, Restorative services, monthly medical director rotation, weekly charge nurse shift.
Key Responsibilities
  • Assess, implement, and coordinate a plan of care for residents.
  • Ensure medications and treatments are administered to assigned residents.
  • Assist with plan of care and interventions for effectiveness and assess for accuracy of the MDS.
  • Provide residents with opportunities for independence and maintain self‑esteem, privacy, and confidentiality.
  • Assure physicians' orders are processed and physicians notified in a timely manner while documenting observations and care given to residents.
  • Direct and assist with resident, family, and significant other education in conjunction with the care team.
  • Evaluate outcomes based on integration among established clinical, financial and utilization data.
  • Utilize independent nursing judgment and appropriate age‑specific structured care methodologies.
  • Interview potential residents and assess residents in compliance with regulations, policy and procedure.
  • Use assessment information for admission and continued stay in the long‑term care community.
  • Ensure all residents meet assisted living qualifications as identified in the occupancy agreement, if applicable.
  • Establish a service plan for each resident and evaluate on an ongoing basis.
  • Communicate with residents, family members and others on the health care team; coordinate, facilitate, and negotiate with others.
  • Monitor, evaluate and trend resident responses utilizing structured care methodologies.
  • Fiscally responsible; control variances and duplication.
  • Work primarily day hours, scheduled weekends, and occasional evenings.
  • Be prepared for multiple sensory and environmental stressors.
  • Other duties as assigned.
Qualifications
  • Bachelor’s degree in Nursing strongly preferred; graduate from an accredited nursing program such as CCNE, ACEN, or NLN CNEA.
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where practice occurs and/or possesses multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtain and maintain required department‑specific competencies and certifications.
  • Wound care certification must be obtained within 1 year of hire.
  • Other certifications and competencies as listed (e.g., QAPI participation).
