Home Health RN Registered Nurse (PRN) – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health RN to provide skilled nursing care for patients in their homes throughout north Hillsborough County (Lutz, New Tampa, Tampa Palms, Temple Terrace, University, Thonotosassa, and surrounding areas). In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Home health visits in north Hillsborough county including:
Lutz, New Tampa, Tampa Palms, Temple Terrace, University, Thonotosassa, and surrounding areas.

PRN role offering 5-10 visits per week. Required weekend/holiday on-call rotations. Potential for full-time in the future if desired!

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
