- JOB PURPOSE:
RN Navigator — Communicates and educates patients/residents and families on disease processes, clinical interventions, and expectations. Promotes customer service. - Educates residents and/or families about aging, pathologies related to older persons, rehabilitation and palliative care; education begins on admission and continues throughout the stay.
- Serves as liaison between resident/family and the healthcare center.
- Participates in care plan meetings and encourages family involvement; facilitates the 72-hour post-admission family conference.
- Participates in weekly Case Mix Meetings and provides timely updates to family members.
- Reviews 24-hour report and conducts appropriate family contact and follow-up; addresses complaints promptly; maintains grievance log per policy with focus on follow-up and resolution.
- Uses checklists, care guard logs, and quality indicators to determine interventions; communicates with resident and family about interventions, progress, and expectations.
- Manages Customer Satisfaction Survey processes and shares results with leadership; follows up as needed.
- Maintains contact with resident/family during hospitalizations via visits, calls, or cards.
- Implements and maintains patient scheduling with ADL Coordinator.
- Maintains knowledge of center policies, procedures, and applicable regulations.
- Participates in surveys and mandatory reports; attends continuing education and in-services to maintain licensure/certification.
- Honors patients’ rights, privacy, and civil rights; complies with corporate compliance.
- Reports occupational hazards; follows safety regulations and equipment procedures.
- Leads culture of excellence through education, competency assessment, and team-building; uses Relias LMS for records.
- Plan, develop, implement, and evaluate orientation, in-services, and education programs; ensure competency evaluation of nursing partners.
- Coordinate training programs (BLS, Wound Care, Tracheostomy Care, Infusion Therapy) and collaborate with NATP instructors; provide hands-on instruction and coaching.
- Manage partner education files and monitor licensure/certification status; use evaluation feedback to improve programs.
- Coordinate education resources and clinical experiences;
Infection Preventionist duties where appropriate; coordinate Employee Health Program. - Establish infection control programs, conduct surveillance, and train staff in hand hygiene and safety.
- Participate in Quality Assurance & Performance Improvement (QAPI) and professional development activities.
- Assist with recruiting, interview participation, and competency assessments; train preceptors and oversee the preceptor program.
- Communicate with leadership about orientation, competency, and education needs; align with DHS outcomes and learning plans.
- Serve as Pruitt University system administrator for the facility; promote ongoing use of the education tool.
- Develop facility orientation and training; communicate policies and ensure policy adherence.
- Maintain confidentiality of health information; demonstrate clinical and teaching competencies; stay current with nursing documentation and Matrix Care.
- Minimum Education:
Bachelor of Science in Nursing or ADN with current enrollment in a BSN program. - Minimum Experience:
Experience implementing policy-driven practices; experience with state/federal regulations; prior education experience encouraged. - Minimum Licensure/Certification:
Current and unrestricted RN licensure; current CPR (BLS); within 90 days complete CMS/state infection prevention training;
Train-the-Trainer NATP certification; within six months complete IV Certification and Tracheostomy Care; obtain American Heart Association CPR Instructor Certification. - Additional Qualifications (Preferred): CPR Instructor, certification by nationally accredited nursing organization (e.g., Gerontological Nursing, CRNI, or Certified Nurse Educator), Pain Management and Wound Care Certification; engagement in scholarly work.
