Overview
The On Call LPN / RN provides direct patient care for patients with life limiting illnesses. The patient population can include newborns through geriatric patients. Uses the Nursing Process to manage the Plan of Care based on nursing standards, policies and procedures. Manages patient care by delegating duties to team members to ensure optimal outcomes. Maintains personal development to enhance nursing practice.
Performs other duties as assigned.
- Utilize all aspects of the Nursing Process (assessment, planning, implementation, evaluation) to assess patients for eligibility for agency programs and to manage the Admission Plan of Care.
- Provide education to patient and family according to the Plan of Care to promote comfort and support in life limiting illness. Reinforce education as needed.
- Document and communicate all required information in accordance with agency policies, accreditation, regulation, and legal requirements.
- Nurse facilitates delegation: RNs delegate specific patient care duties to LPNs and NAs; LPNs work under the direct supervision of the RN.
- Continuously develop professionally.
- Organize and perform work effectively and efficiently.
- Maintain and adjust schedules to enhance program performance.
- Demonstrate positive interpersonal relations with all team members.
- Work independently while managing hospice patient care.
- Maintain and promote customer satisfaction.
- Demonstrate the mission, vision, and values of Lower Cape Fear Life Care daily.
- Education:
Graduate of an approved nursing program (AD, Diploma, or BSN). - Licensure/
Certifications:
Current RN/LPN license in North Carolina or South Carolina. - Experience:
Two years, hospice or home care experience preferred. - Technical/Motor
Skills:
Good hand/eye coordination; ability to use a computer; clear communication with patients, families, physicians, and staff;
English literacy. - Interpersonal
Skills:
Ability to develop positive interactions with patients, families, physicians, and staff. - Physical Requirements:
Ability to lift/move objects up to 45 pounds; ability to assist with patient movement; capable of pushing and lifting as required for patient care. - Mental Requirements:
Ability to assess conditions, formulate plans of care, implement interventions; strong memory and mental faculties. - Sensory Requirements:
Ability to assess patient condition using sight and hearing to evaluate plan of care and communicate with stakeholders. - Hazards/Exposure:
Frequent exposure to blood and body fluids, infectious diseases, needle puncture; may encounter hazards during home visits; temperature and weather extremes. - Hours of Work:
On-call hours:
Friday 5:00 p.m. through Monday 8:00 a.m.; holiday on-call 8:00 a.m. to 8:00 a.m. on designated agency holidays; 48 of 52 weekends per year; all agency holidays; no back-to-back weekends off. - Driving:
May require driving; reliable transportation; subject to Safe Driver Policy. - Population Served:
Infants through geriatrics with life-limiting illness; interactions with physicians, nurses, administrators, discharge planners, social workers, and other health professionals.
