Job Summary
The On Call LPN/RN provides direct patient care for patients with life limiting illnesses. The patient population ranges from newborns to geriatric patients. The nurse is responsible for using the Nursing Process to manage the Plan of Care for each patient based on appropriate nursing standards, policies, and procedures, and for delegating patient care duties to other team members to assure optimal outcomes.
The On Call Nurse also commits to continuing personal development to enhance practice and the profession of Nursing.
Job Duties
- Utilizes all aspects of the Nursing Process (assessment, planning, implementation, evaluation) to assess patients for agency program eligibility and manage the Admission Plan of Care.
- Provides appropriate patient education to the patient/family according to the Plan of Care to facilitate comfort and support in life limiting illness.
- Appropriately documents and communicates all required information in accordance with agency policies, accreditation, regulation, and legal requirements.
- Delegates specific patient care duties to LPNs and NAs; LPNs work under RN supervision.
- Continuously engages in personal development to enhance practice and profession.
- Organizes and performs work efficiently.
- Adjusts schedule to enhance program performance.
- Demonstrates positive interpersonal relations with all team members.
- Works independently while managing hospice patient care.
- Promotes customer satisfaction.
- Demonstrates the mission, vision, and values of Lower Cape Fear Life Care daily.
- Education: Graduate of an approved nursing program (AD, Diploma, or BSN).
- Licensure/
Certifications:
Current North Carolina or South Carolina professional nursing license. - Experience: Minimum 2 years; preference for hospice, home care, or related oncology experience.
- Essential Technical/Motor
Skills:
Hand/eye coordination for injections and computers; clear communication in English. - Interpersonal
Skills:
Positive interaction with patients, families, physicians, and staff. - Essential
Physical Requirements:
Ability to lift or maneuver objects up to 45 pounds; moving and lifting patients. - Essential Mental Abilities: Ability to assess condition, formulate and implement Plan of Care with memory and mental faculty.
- Essential Sensory Requirements: Use of sight to implement and evaluate Plan of Care; hearing to auscultate lung/bowel sounds and communicate.
- Exposure to Hazards: Frequent exposure to blood, body fluids, infectious diseases, needle puncture wounds, hazardous animals, traffic hazards, potential personal safety threats, temperature changes and weather extremes.
- Hours of Work: RN/LPN weekend on‑call 5:00 p.m. Friday – 8:00 a.m. Monday. Holiday on‑call 8:00 a.m. – 8:00 a.m. on designated agency holidays. Commit to 48 of the 52 weekends per year and all recognized agency holidays; no back‑to‑back weekends off.
- Driving: May be required to drive; reliable transportation required; subject to Safe Driver Policy.
- Population Served: Patients of all ages with life limiting illness; includes collaboration with physicians, RNs, facility administrators, discharge planners, social workers, insurance specialists, general public, community leaders, and vendors.
