Functional Title: Licensed Vocational Nurse IV
Job Title: Licensed Vocational Nurse IV
Performs highly advanced vocational nursing work. Provides care, treatment, and general welfare of individuals residing at the Lubbock State Supported Living Center with complex medical, mental health, or behavioral health needs. Trains, models, coaches, and oversees the work of other LVNs. Performs basic and advanced nursing tasks, collects specimens for lab work, and conducts health screenings. Instructs direct support professionals and staff on medical care requirements.
Performs nursing procedures in accordance with facility and state regulations. Oversees on-the-job training for new LVNs and direct care staff. Monitors nursing intervention outcomes and collaborates with the healthcare team. Assists RNs with protocols and programs to improve health and safety. Works under limited supervision of an RN, with substantial initiative and independent judgment.
Attends work regularly and may work specific or rotating shifts, including overtime, as required by policy. Performs complex nursing procedures (e.g., tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments), administers medications, and aids in patient rounds across SSLC locations. Documents treatments and observations in the medical record as required. Trains LVNs and staff on procedures and documentation. Responds to emergencies and contributes to infection control and QA initiatives.
Participates in disaster response and COOP activities as needed.
Skills and Abilities
(KSAs)
Knowledge of nursing care standards, procedures, and clinical decision-making. Knowledge of emergency procedures and equipment (e.g., CPR) and ability to respond in emergencies. Knowledge of state and federal laws and ICF-IID rules relevant to SSLCs. Skill in problem solving, coaching, and training others. Skill in computer use and electronic health records. Ability to organize, coordinate, and evaluate nursing activities; interpret policies/regulations;
recognize medical necessity; and supervise others. Ability to communicate effectively, both orally and in writing.
Must maintain current CPR certification and have the physical ability to safely perform CPR.
Registrations, Licensure Requirements or CertificationsLicensed to practice as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in Texas or in a state that recognizes reciprocity via the Nurse Licensure Compact.
Initial Screening CriteriaGraduation from an LVN or RN program. Minimum 12 months of nursing experience. Experience working with persons with intellectual or developmental disabilities is preferred. Experience in training/leading others is preferred.
Additional InformationApplicants must pass fingerprint background checks, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, and LEIE). Males aged 18–25 must be registered with Selective Service. All State Supported Living Center employees are subject to random drug testing. Flexibility in hours; overtime and weekend/holiday work may be required. Applicants must be at least 18 years old. Telework may be eligible per policy and supervisor needs.
