Case Manager, RN – Clinical Oversight & Patient Care

As our Case Manager, RN, you will play a key leadership role in ensuring high-quality, patient-centered care, while remaining connected to the bedside. This position blends clinical oversight, care coordination, and quality monitoring with direct patient interaction, making it ideal for an RN who values both leadership and hands-on nursing.

Schedule & Compensation
  • Monday–Friday, 8:00 AM–5:00 PM
  • Some after-hours and weekend responsibilities
  • Salary range: $70,000–$80,000

How You Will Make a Difference

In this role, you will:

  • Oversee patient cases from admission through discharge, ensuring care plans are clinically sound, compliant, and individualized
  • Monitor quality of care delivery, including documentation, outcomes, and adherence to clinical standards
  • Maintain active involvement in bedside nursing, completing select patient visits to stay clinically connected and support care continuity
  • Serve as a clinical resource and leader for nurses and caregivers, offering guidance, education, and support
  • Facilitate weekly case management meetings to review patient progress, identify risks, and improve care strategies
  • Collaborate closely with leadership, referral sources, and interdisciplinary partners to ensure seamless communication and optimal patient outcomes
  • Identify opportunities for process improvement and contribute to the ongoing growth of clinical excellence within the organization

This position is well-suited for an RN who wants to step into leadership without stepping away from patient care.

What’s Required to Join Our Team
  • Active RN license
  • Minimum 2 years of nursing experience
  • Strong clinical judgment with a detail-oriented approach to patient care and documentation
  • Excellent interpersonal and communication skills—you will interact with patients, families, clinicians, and referral partners daily
  • A collaborative mindset with an interest in mentorship, quality improvement, and care coordination
