BriefJob Description
This position is available for Morning, Evening and Night shifts on multiple homes. It may be eligible to earn additional pay for work performed on evenings, nights and/or weekends. The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state.
Key Responsibilities- Perform advanced vocational nursing work for individuals residing at the State Supported Living Center who have complex medical, mental health and/or behavioral health issues.
- Execute basic and advanced routine and non-routine nursing tasks to promote health of individuals within assigned areas and shifts.
- Collect specimens for lab work, perform health screenings, and administer medications and treatments in various modalities.
- Provide instruction and training to direct support professionals, new LVNs, and other staff on care procedures and medical needs.
- Maintain compliance with agency, SSLC, and unit/home policies and state regulations.
- Monitor patients’ vital signs, weight, diet and feeding, and assess health status post-treatment.
- Document and report treatments and observations in the electronic health record or paper chart accurately and timely.
- Collaborate with multidisciplinary team members to evaluate nursing intervention outcomes.
- Assist RNs with implementation of protocols, procedures and quality improvement programs.
- Participate in emergency response, infection control, and safety training as required.
- Perform additional duties as assigned, including disaster response or COOP activation.
- Attend work regularly and may work rotating or overtime shifts per leave policy.
- Serve in an on-call status as needed to meet facility operational needs.
- Administer medications and treatments orally, rectally, internally, or by injection.
- Perform complex technical nursing procedures such as tracheostomy care, gastrostomy/jejunostomy care, wound care, and breathing treatments.
- Collect and manage specimens, conduct screening tests, and notify RN or Primary Care Provider of illnesses.
- Promote interdisciplinary collaboration and record all pertinent information for the treatment team.
- Lead, monitor, and oversee others in monitoring patients during care activities.
- Train and supervise new LVN staff, monitor their performance, provide feedback, and assist in job‑training checklists.
- Assist in infection control, QA nurse, and therapeutic nursing program implementation.
- Respond to emergencies, maintain safety protocols, and provide staff safety training.
- Participate in disaster response, continuity operations, and other assigned tasks.
