The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus‑based direct services and supports at 13 centers located throughout the state.

Responsibilities
  • Perform complex nursing work providing for the assessment, care and integrated treatment of individuals residing at the SSLC.
  • Oversee the delivery of nursing care necessary and monitor the health care status to meet the health care needs of individuals assigned to the caseload.
  • As a member of the interdisciplinary team (IDT), work closely and professionally with members of other disciplines to ensure that communication is in place to provide consistent, high quality interdisciplinary and holistic care for all persons served.
  • Perform physical assessments, document healthcare needs, develop health care plans, oversee the implementing nursing intervention(s), evaluate and analyze the outcomes of the interventions.
  • Work proactively to ensure optimal health care outcomes for assigned individuals.
  • Review available documentation for issues and incidents involving assigned individuals.
  • Perform and report trend analysis of health care needs and/or issues to the appropriate person and create nursing care plans that accurately and definitively direct the implementation of all necessary care.
  • Self‑initiate to maintain competence in nursing best practices.
  • Use established nursing methods, procedures and techniques to successfully complete job duties.
  • Attend medical appointments with individuals when need to ensure optimal communication and continuity of care.
  • Participate in quality assurance reviews, attend meetings, mentor and coach new RN case managers as assigned, and participate in committees and work groups as assigned.
  • Provide treatment to HHS employees injured in the course and scope of employment as needed.
  • Work under the general supervision of a manager who is a Registered Nurse, with latitude for the use of initiative and independent judgment.
Essential Job Functions
  • Attend work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
  • May serve in an on‑call status on a rotating schedule as required to meet operational needs.
  • Monitors the health status of all individuals assigned to the RN Case Manager.
  • Conducts rounds on the assigned home(s) to gather information from direct support professionals, nursing staff, and others.
  • Observes and interacts with the individuals.
  • Oversees the implementing nursing intervention(s), evaluates and analyzes the outcomes of the interventions.
  • Uses nursing skill and judgment and established procedures to identify current and emerging issues.
  • Documents and follows up on those issues in accordance with protocols.
  • Ensures that proactive nursing care is provided in an attempt to promote the optimal health of persons served.
  • Demonstrates thorough knowledge of the nursing process and nursing procedures.
