The Facility Investigator, Investigator III, performs moderately complex (journey‑level) investigative work at the Lubbock State Supported Living Center (SSLC). They conduct field investigations of Abuse, Neglect, and Exploitation (ANE) and injuries Bayer residents, perform comprehensive preliminary investigations to ensure protection from harm and reporting guidelines are followed, and follow‑up on outstanding external cases such as those through HHSCPI and the Office of Inspector General (OIG) to ensure documentation guideline dates are adhered to.

They work with Campus Administrators to ensure incidents that occur outside business hours remain in compliance with ICF and Settlement Agreement requirements, verify information, collect and preserve evidence, data and documents, review evidence, evaluate findings, prepare reports, and present testimony and evidence at administrative hearings or court proceedings. They serve on the Incident Management Team, conduct root cause analysis meetings, complete data entry for unusual incidents, assist with tracking injury reports, consult with State Office Incident Management, and provide technical assistance and administrative support to accomplish the State Operated Facilities missions.

Flexibility in work schedule is necessary, including being on call for emergency investigation work.

Essential Job Functions (EJFs)

Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime. Conducts investigations of Reportable Incidents and Serious Injury and the preliminary investigation of unusual incidents to ensure prompt intervention in the care, safety and well‑being of persons served. Collects and reviews evidence and data through field investigations and data collection, including direct observations and review of video surveillance.
