The Clinical Care Coordinator is responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the medical group. Daily work includes ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized. The Clinical Care Coordinator assures that an integrated approach to the patients' care coordination is fulfilled.
This position is the liaison between providers, primary care clinics, specialist offices, patients, multiple Providence St Joseph Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for health programs and related processes.

Required Qualifications- 2 years work experience in a healthcare setting.
- Experience with electronic medical records and/or computerized billing systems.
- Epic or other Electronic Health Record /care management software experience.
- Healthcare certification/license.
- 2 years work experience in an outpatient/clinic/ambulatory setting.
