Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.

Responsibilities
  • Maintains a working knowledge of community resources;
  • Coordinates referrals to community agencies and tracks outcomes of referrals;
  • Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients;
  • Documents all client encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client;
  • Reviews encounters and records of community prevention workers;
  • Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community;
  • Coordinates organizational and community groups that are working to address social determinants of health;
  • Develop and implement educational programs;
  • Assists with the development of policy and procedures;
  • May supervise or coordinate the work of community workers;
  • Plan goals and objectives for assigned programs;
  • Assist in budget preparation;
  • Assist with health department social media;
  • Additional duties as assigned.

Valid Texas Driver's License Required.

Qualifications

Experience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred.

Knowledge and Abilities
  • Knowledge of local community resources for social service, mental health and healthcare;
  • Skill in developing, implementing and monitoring service and treatment plans;
  • Knowledge of current principles and practices of planning and program implementation;
  • Knowledge of organization and administration of public health and/or human service programs;
  • Knowledge of program monitoring and policy analysis;
  • Skill in communicating effectively orally and in writing;
  • Skill in interviewing;
  • Skill in organizing and developing programs independently;
  • Skill in establishing effective and constructive working relationships;
  • Skill in use of personal computers and related software applications, including e-mail;
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community;
Physical Requirements
  • Occasionally stand and walk;
  • Frequently flex upper truck forward;
  • Occasionally lift up to 10 pounds;
  • Occasionally push and pull objects;
  • Occasionally flex upper trunk forward, partial flexing o knees;
  • Occasionally reach above, at or below shoulder height;
  • Continuously talk, see and hear.
