Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.
Responsibilities- Maintains a working knowledge of community resources;
- Coordinates referrals to community agencies and tracks outcomes of referrals;
- Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients;
- Documents all client encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client;
- Reviews encounters and records of community prevention workers;
- Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community;
- Coordinates organizational and community groups that are working to address social determinants of health;
- Develop and implement educational programs;
- Assists with the development of policy and procedures;
- May supervise or coordinate the work of community workers;
- Plan goals and objectives for assigned programs;
- Assist in budget preparation;
- Assist with health department social media;
- Additional duties as assigned.
Valid Texas Driver's License Required.
QualificationsExperience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred.
Knowledge and Abilities- Knowledge of local community resources for social service, mental health and healthcare;
- Skill in developing, implementing and monitoring service and treatment plans;
- Knowledge of current principles and practices of planning and program implementation;
- Knowledge of organization and administration of public health and/or human service programs;
- Knowledge of program monitoring and policy analysis;
- Skill in communicating effectively orally and in writing;
- Skill in interviewing;
- Skill in organizing and developing programs independently;
- Skill in establishing effective and constructive working relationships;
- Skill in use of personal computers and related software applications, including e-mail;
- Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community;
- Occasionally stand and walk;
- Frequently flex upper truck forward;
- Occasionally lift up to 10 pounds;
- Occasionally push and pull objects;
- Occasionally flex upper trunk forward, partial flexing o knees;
- Occasionally reach above, at or below shoulder height;
- Continuously talk, see and hear.
