We are seeking an experienced and motivated Lead Practice Nurse to join our forward thinking and patient focused primary care team. AsLead Practice Nurse, you will play a key leadership role in the delivery and development of high-quality nursing services within the practice.

You will be responsible for overseeing the nursing team, developing staff potential,and motivating your team through mentoring, appraisals and learning events. Youwill contribute to quality improvement initiatives and ensure clinical excellence.

This is a pivotal role that combines hands-on clinical practice,leadership and service co-ordination. While previous leadership experience is desirable,we also welcome applications from experienced practice nurses who are ready totake the next step in their career. For the right candidate this can be adevelopmental role with support to transition into leadership.

Main duties of the job

Provide clinical leadership & direction to the nursing team, ensuring high standards of patient care are consistently delivered. Supervise, mentor and support the practice nursing team, identifying training and support needs tosupport ongoing professional development.

Coordinate and lead the delivery of a wide range of nursing services, including chronic disease management (e.g. diabetes, asthma, COPD), immunisations, woundcare, cervical screening, and health promotion.

Ensure compliance with current clinical guidelines, infection control policies,and national/local protocols. Contribute to audits & risk assessments

Deliver holistic, evidence-based care tailored to individual patient needs,ensuring safeguarding, dignity, and respect at all times.

Work collaboratively with GPs, other clinical staff, and external health andsocial care partners to ensure integrated, high-quality care.

Maintain accurate, comprehensive clinical records using appropriate systems (Systm One),ensuring data protection and confidentiality.

Support the development and implementation of practice policies, protocols, and procedures in line with best practice and regulatory requirements.

Take a lead role in managing and reviewing patients with long-term conditions,using data and recall systems to support proactive care.

Job responsibilities

Support the development of others to maximise staff potential in the team.

Identify the potential for service developments, risk and deficits and inform practice manager making recommendations based on specialist knowledge

Undertake one-to-ones, appraisals, regular Practice Nurseand HCA meetings, clinical education, and other practice-wide learning eventsand meetings.

To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

As the Clinical lead, you will have overall line management responsibility for the Practice Nursing Team

Play a key role in our clinical governance and overall practice management team.

Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority.

Attend regular practice meetings.

Lead the nursing team in the planning and implementation oflocal guidelines, protocols, and standards, as well as local projects or initiatives.

Ensure health and safety and infection control are high priorities, supported by other members of the team.

Work with our Practice Management team to ensure we monitor practice performance of QOF and LCS regularly to achieve high standards of patient care.

Take responsibility for own development, learning, and performance, including organising clinical supervision where appropriate andacting as a positive role model.

Work to support staff wellbeing and development.
