Job Summary: The Chronic Registered Nurse is accountable for providing and coordinating clinically competent nursing care for patients undergoing chronic hemodialysis treatment. As a key member of the End Stage Kidney Disease (ESKD) care team, this role supports patient assessment, care planning, education, and quality improvement initiatives to enhance patient outcomes and facility operations.
PrincipalDuties & Responsibilities
- Provide comprehensive pre-, intra-, and post-hemodialysis nursing care for assigned patients.
- Perform ongoing assessment, analysis, documentation, and modification of patient treatment plans as clinically indicated.
- Collaborate with physicians, supervisors, charge nurses, and interdisciplinary team members to ensure safe and effective care delivery.
- Administer medications per prescription or approved protocols and document clinical effectiveness and justification.
- Educate patients and families on treatment plans, transplant options, and modality choices.
- Direct and support patient care staff, including RNs, LPNs/LVNs, and Patient Care Technicians, as appropriate.
- Initiate and assist with emergency response measures when required.
- Ensure proper laboratory collection, processing, shipping, and rescheduling of missed or insufficient samples.
- Participate in staff training, orientation, leadership activities, and quality improvement initiatives.
- Maintain accurate and timely patient medical records.
- Perform all other duties as assigned by the Supervisor.
May serve as Team Leader or Nurse in Charge upon meeting all requirements, including:
- Completion of all required education and training
- Minimum 9 months RN experience
- Minimum 3 months chronic or acute hemodialysis RN experience
- Graduate of an accredited School of Nursing
- Current and unrestricted state RN license
- CPR / BLS Certification (current or upon hire)
- Must meet all state practice requirements
- 1 year of nephrology nursing experience within the last two years as an RN
- Chronic hemodialysis experience required
- Strong clinical judgment, documentation, and patient education skills
- Ability to work collaboratively in a multidisciplinary healthcare environment
- Frequent standing, bending, and direct patient care activities
- Regular lifting, moving, and assisting patients with ambulation
- May lift up to 30 lbs. and assist with equipment up to 200 lbs. (with assistance)
- Exposure to infectious and contagious diseases/materials
- Work performed in a healthcare facility environment with controlled temperatures and moderate noise
- May require travel to training sites or other facilities
- Successful completion of Ishihara Color Blindness Test required (reasonable accommodations considered)
