ESSENTIAL DUTIES AND RESPONSIBILITIES

Participate with interdisciplinary team to plan, coordinate, and evaluate residents’ care. Provide direct care and/or assist with care as directed, consistent with scope of practice and competency. Initiate the SBAR process and collaborate with the RN for assessment and recommendations.

Document resident condition and nursing needs accurately and promptly. Report observations and reactions to RN.

Provide accurate and complete resident status reports at shift change and maintain the 24‑hour report book.

Receive and record physicians’ orders.

Develop work assignments for Nurse Aides and provide feedback to Unit Managers regarding performance.

Report problems and complaints promptly to Unit Manager or Supervisor.

Assist with data collection for admission, transfer, and discharge of residents and complete clinical paperwork.

Serve on and participate in committees as assigned.

Maintain inventory and supplies in a clean, safe manner and communicate needs to housekeeping, maintenance, or nutritional services.

Support and participate in the center’s performance improvement initiatives and survey readiness.

Make rounds with physicians and other team members as needed; assist physicians and consultants with special tests or procedures within scope of state nurse practice act.

Participate in resident assessment and care planning activities and revise plans as needed per state nurse practice act.

Coordinate residents’ care activities and communicate changes to team members and resident family/significant other.

Administer treatments and mediate emergencies, including CPR.

Document care performed and observations in clinical records promptly following standard guidelines.

Monitor completeness and accuracy of own electronic medical record entries and those of assigned LPN and Nurse Aides daily.

Participate in medication management by administering medications as ordered, observing resident responses, identifying adverse reactions, maintaining narcotic records, and completing medication interchange and insurance as assigned.

Initiate and assist consultants while providing services; review documentation and follow up on recommendations.

Assist with follow‑up on results of qualitative and quantitative medical record audits; communicate and implement pharmacy review recommendations as assigned.

Provide individual and unit education to residents/families and staff within scope of practice.

Orient new employees.

Commit to working with residents and families to develop and implement person‑centered care strategies.

Apply “5 Star” customer service approach in all job aspects.

Adhere to all company policies, procedures, and standards of conduct.
