As a Chronic Registered Nurse
, you’ll play a vital role in the care of patients with End Stage Kidney Disease (ESKD). This position is responsible for coordinating and delivering clinically competent care, including assessment, planning, intervention, and evaluation. The RN works closely with patients, families, and interdisciplinary teams to drive high-quality outcomes and continuous improvement in facility operations.
- Perform all essential dialysis nursing functions under supervision and alongside other RNs
- Assess patients before, during, and after treatment and update care plans as needed
- Educate patients and families about treatment plans and care requirements
- Administer prescribed medications and monitor for effectiveness
- Lead or assist in emergency response situations
- Support the training and onboarding of new staff
- Ensure accurate laboratory collection, processing, and shipping
- Collaborate with interdisciplinary teams to identify care outcomes and maintain care plans
- Educate patients on transplant and other treatment options
- Carry out other duties as assigned
- At least 1 year of Nephrology Nursing experience within the last 2 years
- Chronic hemodialysis experience required
- Minimum of 9 months RN experience and 3 months dialysis RN experience
- Experience as Charge Nurse or in leadership positions
- Familiarity with transplant and dialysis education
- Hemodialysis care
- Patient assessment and education
- Medication administration
- Emergency response
- Staff mentoring or preceptorship
- Quality improvement involvement
- Electronic Medical Records (EMR) proficiency
- Team-based collaboration
- CPR/BLS Certification
- Ability to lift and move equipment up to 200 lbs. (with assistance)
- Flexibility to travel between facilities
- Standing and bending for prolonged periods
- Compliance with Ishihara Color Blindness testing (accommodation available)
