Overview
The ACCS Assistant Team Leader serves as a primary resource for management, clinical expertise and staff development of an Adult Community Clinical Services (ACCS) Team. The ACCS Teams are interdisciplinary teams of outreach workers designed to provide comprehensive clinical and rehabilitative services to adults with psychiatric conditions. Working under the ACCS Team Leader, the ACCS Assistant Team Leader monitors engagement of all Persons Served (PS), selects intervention strategies for those who are difficult to engage, and designs strategies to reduce barriers to living independently in their communities.
The ACCS Assistant Team Leader trains, mentors and models clinical evidence-based practices to all team members and to staff based in Group Living Environments and Supported Independent Environments (GLE/SIE).
Job duties and responsibilities include but are not limited to:
- Assist in the oversight of clinical programming and service delivery by the team to ensure the needs of Persons Served are met.
- Provide clinical supervision to clinically licensed and other assigned staff.
- Provide competency-based staff training on clinical evidence-based practices to ACCS team members and GLE/SIE Program Directors.
- Consult with licensed staff on appropriate clinical interventions for treatment plans, care transitions, and crisis prevention tactics.
- Function as a primary liaison with the Department of Mental Health (DMH).
- Perform other related duties as assigned.
- Oversee clinical programming and service delivery by the team.
- Monitor engagement status of all PS, consult on intervention strategies, and support LPHAs in addressing engagement barriers to prevent discharge due to disengagement or homelessness.
- Collaborate with LPHA and Lead Peer Specialist on orientation, engagement and enrollment activities for PS with high risk behaviors.
- Provide competency-based staff training on clinical evidence-based practices and best practices to team members and GLE/SIE Program Directors, including Motivational Interviewing;
Harm Reduction and Addiction Treatment; SBIRT;
Housing First;
Stages of Change;
Seeking Safety;
Cognitive Behavioral interventions;
Safety Plans;
Crisis Prevention and Response Plans; 8 Dimensions of Wellness; and documentation standards. - Mentor and model best practices and evidence-based interventions for all staff.
- Support staff use of trauma-informed, recovery-oriented, person-centered, culturally competent and strengths-based approaches and skills training.
- Provide performance support and supervision, including competency-based assessments and field supervision of LPHA and Licensed staff interacting with PS.
- Promote access to healthcare, employment and education services for PS.
- Encourage engagement, housing permanency, community tenure, greater self-sufficiency and successful completion of services.
- Consult with LPHA and Licensed staff on interventions for difficult-to-engage PS.
- Review LPHA treatment plans for effectiveness and ensure timely documentation of PS needs and services.
- Oversee clinical risk assessment and management strategies; meet with PS as needed for assessments and participation negotiations.
- Consult with LPHA as needed to complete Safety Plans and Crisis Prevention and Response Plans for PS.
- Select treatment interventions for high-risk PS and determine high-risk alert status.
- Lead team efforts in crisis prevention and intervention for high-risk PS.
- Provide training to all team staff on risk management and safety procedures.
- Provide on-site consultation to GLE/SIE staff with LPHA to address engagement, risk and clinical needs of PS and integrate risk management processes.
- Manage challenging care transitions with LPHA for high-risk PS.
- Serve as primary liaison with DMH Legal Office regarding Guardianship; manage Guardianship applications, status, and related paperwork; coordinate with the Team Psychiatrist on competency evaluations; communicate legal scope and clinical implications of guardianship terms.
- Serve as liaison to DMH risk management and Care Coordination entities for high-risk PS.
- Notify DMH Site Office of significant events and critical incidents; manage adverse events and corrective actions.
- Request Risk Management assistance from DMH as needed.
- Request additional testing or resources as needed for high-risk PS, including competency evaluations; inform DMH regarding planned discharges and pre-discharge outreach.
- Ensure adherence to Vinfen risk mitigation policies and procedures; analyze risk data to inform practice improvements.
- Conduct Treatment Planning meetings as needed; coordinate with Integrated Team Consulting Psychologist and Occupational Therapist; seek consultations from Vinfen leadership as needed.
- Covers Integrated Team Leader duties during absences; may perform LPHA coverage duties when LPHA staff are absent.
