Job Summary and Qualifications

Job Summary: Case Manager will assist with services to individuals and families experiencing homelessness who have been approved by LMHA to receive Permanent Support Housing/Shelter Plus Care Services. In tandem with other members of the VOA Home team, the Case Manager will assist clients in accessing medical and mental health services, emergency and long-term housing, substance abuse services, and social services.

Case Managers work within a harm reduction and person-centered model.

What you should have for this role

  • This position requires a Bachelor's degree in social work or a related field or at least five (5) years of experience in the field; no degree would qualify for this position. Requires demonstrated ability to provide skills in family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
  • Commitment to the principles of the project. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
  • Personal automobile, valid driver's license, liability insurance, and willingness and ability to travel within the full-service area. Must complete required case management and CTI (Critical Time Intervention) training within 90 days of hire.

Responsibilities
  • Provide outreach to community partners and represent VOA values in interactions with community partners to increase awareness of and recruit landlords to work with our client families in the program.
  • Perform assessment and develop/monitor case plans and conduct necessary follow-up activities.
  • Establish linkages with appropriate agencies and service providers in the area/community.
  • Provide referrals and coordinate with community partners and other service providers to assist participants in accessing health care services and daily living services.
  • Decide how resources are allocated to participants on the basis of need and eligibility, according to grant guidelines.
  • Educate participants on issues such as supportive services availability and participant rights.
  • Provide assistance to participants in obtaining public benefits.
  • Provide assistance to participants in obtaining a birth certificate, driver’s license, or TARC tickets.
  • Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, recovery programs, legal services, and housing counseling for the purpose of overcoming obstacles preventing obtaining stable housing or emergency shelter.
  • Complete required documentation (including progress notes) within 48 hours of contact.
  • Complete and submit the required documentation for entry into the Homeless Management Information System (HMIS) and Electronic Health Record system within 48 hours of entry, and portal/billing logs to the Program Manager weekly.
