Vital Link operates as a distinct organization focused exclusively on employee support and advocacy, partnering internally with care teams and external community resources to remove barriers that impact workforce participation.
OverviewThe Advocate role provides empathetic, non-judgmental support and partners closely with internal Vital Link team members – particularly Lead Advocates – to deliver coordinated, high-quality assistance. The role focuses exclusively on direct employee support and internal care coordination.
Onsite role at our Corporate Louisville Office!
How you Will make a difference- Provide individualized advocacy and support to participants enrolled in the Vital Link program, using a trauma-informed and culturally responsive approach.
- Conduct inbound and outbound communication with participants via text message, email, phone, and virtual meetings to assess needs and coordinate support, responding professionally and empathetically across all channels.
- Navigate and coordinate services across healthcare, behavioral health, financial assistance, housing, legal aid, and other community-based resources.
- Maintain accurate, timely, and confidential documentation of participant interactions and outcomes within Salesforce case management while ensuring each enrollment is tracked as an active case.
- Educate participants on Vital Link services, how to access resources, and how to advocate for themselves when appropriate.
- Collaborate closely with Lead Advocates and other Vital Link team members to coordinate care, escalate complex cases, and ensure continuity of support.
- Coordinate with external service providers and community-based resources as needed to support participant outcomes.
- Respond promptly to participant inquiries while meeting defined service level expectations.
- Identify trends, gaps, or recurring barriers and recommend process or program improvements.
- Uphold all confidentiality, privacy, and data protection requirements.
- Support change initiatives and special projects aligned with Vital Link growth and mission.
- Associate degree or higher in Human Resources, Social Work, Human Services, Sociology, or a related field or equivalent combination of education and experience.
- Minimum of one (1) year of experience in one or more of the following areas: HR or employee relations, social services or case management, workforce support programs, healthcare, behavioral health, or community-based advocacy.
- Familiarity with benefit navigation, leave management, ADA processes, or employee assistance programs.
- Experience partnering with community-based organizations or public assistance programs.
- Demonstrated ability to work with individuals facing complex or sensitive life circumstances.
- Strong verbal and written communication skills, with the ability to communicate professionally and empathetically via text, email, and phone.
- High level of professionalism with demonstrated ability to maintain confidentiality.
- Customer service oriented, with the ability to work well under pressure.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple cases with high level of quality and productivity.

Salary Range
Up to USD $55,000.00/Yr.
