Overview
The Targeted Case Manager coordinates the treatment team for Renewal clients; serves as a liaison and representative for child, family, foster parents, and outside agencies; coordinates services both within Renewal and in the community; monitors quality of services for clients on their caseload; provides crisis management and support. The Targeted Case Manager implements and coordinates treatment or habilitation plans for individual clients in the assigned population group, including evaluation, consultation, education, and follow-up services.
Provides personal case management through direct client contact, assessment, crisis assistance, client family contact, monitoring of client progress, etc. Identifies services available in the community that could potentially benefit clients, arranges for delivery of those services, and assists clients in gaining access to services.
Responsibilities are encompassed in the overview above and include coordinating treatment teams, serving as liaison for clients and families, identifying community services, and monitoring client progress to ensure quality of services.
Qualifications- Master's degree from an accredited university or at least one year of clinical case management experience, or experience working directly with relevant client population with a bachelor's degree from an accredited university in a social services field
- Must possess a valid driver's license and be eligible to drive per the Maryhurst driving policy
- Must possess a reliable source of transportation for frequent travel
- Completion of required case management certification course within six months of date of employment
- Knowledge of applicable population group issues, treatments, and available services
