The Nurse Care Manager is an LPN galvanizing patient‑centered care by focusing on clinical assessments, care planning, and patient education. Working closely with field‑based providers, the Nurse Care Manager assesses patient needs, coordinates care, and ensures clinical care plans are effectively implemented to achieve positive health outcomes.

• Clinical Assessment and Documentation:
  Conduct initial and ongoing clinical assessments of patients, inquire about vital signs, gather all information regarding patient condition or issue, prioritize patient issues, assign risk category, and document all patient care data in the Electronic Health Record (EHR).
• Contribute to the development and updating of individualized care plans in collaboration with the care team and providers.
• Maintain productivity metrics.
• Care Coordination and Patient
  
  Education:
  
  Coordinate with providers, family members, and caregivers to facilitate the implementation of care plans, ensuring all parties are informed and engaged; provide education to patients and caregivers on treatment plans, medication management, and other care needs to improve patient outcomes and adherence.
• Medication and Treatment Management:
  Handle medication‑related inquiries, process prior authorizations, document all interactions with patients regarding medication adjustments; reviewאס and process clinical orders as needed Dharma ensuring compliance with provider instructions and patient needs.
• Compliance and Quality Assurance:
  Ensure all clinical documentation meets regulatory and compliance standards, adhering to HIPAA and company policies; participate in regular care management meetings and quality assurance checks to maintain high standards of patient care.
• Collaboration and Communication:
  Work closely with Care Facilitators for scheduling support and non‑clinical inquiries to focus on core clinical duties; participate in team meetings and provide insights on patient care processes, identifying areas for clinical improvement.

• Associate degree preferred
• Prior experience in a patient facing capacity required
• Familiarity with Care Management preferred
• Experience with Transitional Care Management strongly preferred
• Minimum of 3-years of experience in geriatric or complex patient care
• Experience working with Electronic Health Records (EHR) software and a Health Information Exchange
• Licensed Practical Nurse (LPN) with an active license; compact license preferred.
• Excellent communication skills
• Comfortable assessing status of patient remotely, active listener, motivational interviewing skills
• Abilityрика to recognize condition specific symptoms
• Organized, thorough documentation skills
• Solid time management skills
• Ability to work independently as well as part of a clinical care team
• Ability to develop and build trusting relationships with patients and partners
• Able to use Microsoft Excel for tracking and reporting
