Position SummaryThe Licensed Practical Nurse (LPN) in the Memory Care Unit (Aldersgate) is essential in providing skilled nursing care and support to individuals with memory impairments, such as Alzheimer's disease or dementia. The position works closely with the Resident Services team to ensure the well‑being and safety of residents while promoting their overall quality of life.
Essential Duties and Responsibilities- Administer medications and treatments as prescribed by physicians, ensuring accurate documentation and adherence to medication management protocols.
- Perform routine nursing assessments, including vital signs, physical examinations, and cognitive assessments, to monitor residents' health status and detect any changes or concerns.
- Collaborate with the healthcare team to develop and implement individualized care plans based on residents' needs and preferences.
- Provide direct nursing care, including wound care, catheter care, and assistance with activities of daily living (ADLs) such as bathing, dressing, and eating.
- Communicate effectively with residents, their families, and the interdisciplinary team to ensure a comprehensive and coordinated approach to care.
- Act as a liaison between residents, families, and healthcare professionals, relaying important information, addressing concerns, and providing emotional support.
- Collaborate with other healthcare professionals, such as physicians, therapists, and social workers, to coordinate and integrate care services.
- Document and maintain accurate and up-to-date medical records, including nursing assessments, care plans, and progress notes.
- Implement and enforce safety protocols to maintain a secure and comfortable environment for residents.
- Monitor residents for any signs of distress, changes in behavior, or medical emergencies, and take appropriate action as needed.
- Respond promptly and effectively to medical emergencies, following established protocols and procedures.
- Other duties as required.
- High School Diploma or GED.
- Active Licensed Practical Nurse license within the state of Kentucky.
- Commitment to ongoing education, such as attending workshops, seminars, and pursuing additional certifications or specialized training in memory care.
- Ability to provide compassionate and personalized care to residents.
- Effective and clear communication skills.
- Knowledge of medication management, including proper dosage, administration routes, and potential side effects.
- Attention to detail and adherence to legal and ethical standards.
- Willingness to collaborate and work as part of a multidisciplinary team.
