• Must possess a Licensed Practical Nurse diploma and a valid Licensed Practical Nurse license issued by the Kentucky Board of Nursing or a state Nursing Board in the “Compact State Agreement”.

• Associate Degree in Nursing
• Two (2) years related experience and/or training as a Nurse and at least 1 year with Developmental Disabilities.

• Responsible for ensuring all Hazelwood residents are given ample and sufficient supports and services necessary to ensure they can become an active member within the community which will assist with enhancing their lives.
• Under the clinical supervision of a Registered Nurse, performs nursing delegated duties in the care of ill or injured clients.
• Prepares, administers, and charts medication.
• Performs medical treatment instructed by the physician.
• Makes appointments for medical follow up as directed by physician.
• Gives injections.
