Job Profile Summary

The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in Chen Med/Jencare medical centers, nursing homes, SNFs, and home settings depending on the assignment. Responsibilities include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Chen Med standards for quality, service, productivity and teamwork.

The NP also participates in clinical rounds and conferences and provides in‑depth documentation through written progress notes and summaries.

The NP must demonstrate the ability to function independently and in collaboration with other health care professionals. The position may require participation in risk and quality management programs and clinical meetings.

Job Duties /Responsibilities
  • Independently assess acute and non‑acute clinical problems; perform and document physical assessments and patient histories; analyze trends; develop, document, and implement a patient management plan.
  • Plan patient care based on in‑depth knowledge of the specific patient population; anticipate physiological and/or psychological problems; serve as patient advocate.
  • Write admission, transfers, and discharge orders; order and interpret lab and diagnostic studies; order appropriate medication and treatments.
  • Refer patients for consult when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc.).
  • Document through in‑depth progress notes and summaries.
  • Perform invasive procedures independently upon completion of documented competency.
  • Participate in patient care rounds and conferences; communicate patient management strategies to team members.
  • Recognize situations requiring immediate physician attention and initiate life‑saving procedures.
  • Use advanced communication skills to problem‑solve complex situations and improve processes and service to patients.
Other Responsibilities May Include
  • Collaborate with multidisciplinary team members to analyze and evaluate current systems and implement new practice patterns.
  • Participate in outside activities that enhance personal and professional growth.
  • Initiate arrangements and write orders for SNF discharges and complete appropriate paperwork.
  • Work collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as medically appropriate.
  • Introduce self to patient/family and explain NP role.
  • Facilitate patient/family conferences to review treatment goals and optimize resource utilization.
  • Collaborate to maximize patient/family ability to make informed decisions about goals of care, palliative care and hospice.
  • Facilitate discharge to appropriate level of care; use preferred providers when additional services are required.
  • Prescribe medication to patients based on state of practice.
  • Other duties as assigned and modified at manager’s discretion.

Education and Experience Criteria
  • ARNP or similar advanced degree in Nursing required.
  • Current Nurse Practitioner Certification in the state of practice required.
  • Board certification by AANP or ANCC preferred, may be required for certain states.
  • Current DEA number for schedule II‑V controlled substances may be required based on state of practice.
  • Minimum 2 years of clinical NP experience, preferably in Family Medicine, Internal Medicine, or Geriatrics, including prior autonomy in independent care decisions.
  • Basic Life Support (BLS) certification required within first 90 days of employment.
Pay Range

$221,141 – $315,915 Salary. This range represents the base hourly rate or full‑time annual salary. Final compensation depends on experience, education, location, and other factors. The position may also be eligible for bonuses or commissions.
