We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Immediate Care team at a Federally Qualified Health Center (FQHC). The ideal candidate will provide high-quality nursing care to patients in a fast-paced environment, ensuring their comfort and well-being.

• Provide direct patient care, including assessments, medication administration, and wound care.
• Assist in the development and implementation of patient care plans.
• Monitor and document patient progress and responses to treatment.
• Educate patients and their families on health management and disease prevention.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Maintain a clean and safe environment for patients and staff.
• Adhere to all regulatory and organizational policies and procedures.

• Current LPN license in the state of practice.
• Previous experience in an immediate care or urgent care setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Proficient in electronic health record (EHR) systems.
• Commitment to providing culturally competent care to diverse populations.
