Responsibilities
The Substance Use Support Coordinator (SUSC) provides support to patients who use substances, with the goal of increasing the health, wellbeing, and safety of patients and members of the patient’s care team. Conducts the initial assessment to determine patient insight into impact of their substance use across dimensions of their life, identify recent patterns of use, as well as identify behavioral changes patient is ready to make.
Works collaboratively with other hospital staff to reduce barriers to patient engaging in medical care, helps patient successfully navigate hospital admission, and connects patient with SUD treatment/therapy and other community resources. All services provided will align with a person-centered and patient-directed approach to providing care.
Required:
- One year in harm reduction or substance use disorder within public health, healthcare setting, or similar setting in a paid or unpaid status
Desired:
- Three years in harm reduction or substance use disorder within public health or healthcare setting
- High School Diploma or GED;
Bachelor Degree - Registered Alcohol and Drug Peer Support Specialist
