We are seeking applicants for Community Living Support Professional to assist individuals with developmental or intellectual disabilities in their daily living activities within their community. The primary role is to provide support and guidance to help these individuals lead fulfilling and independent lives. The role may involve assisting with personal hygiene, meal preparation, medication management, transportation, household chores, and accessing community resources and services.
Community Living Support Professionals work closely with clients, their families, and other healthcare professionals to create personalized care plans and ensure the well-being and inclusion of clients within the community. They promote independence, autonomy, and quality of life for individuals with disabilities.
$16.00/hr - $16.00/hr

- Must be 18 years of age
- Must have a high school diploma or its equivalent
- Must have at least one year of experience working with adults with developmental and/or intellectual disabilities
- Must complete all required training as outlined by DDID regulations, including first aid and CPR certification
- Must be able to read, understand and implement written and oral instructions
- Must pass a criminal background check
- Must pass pre-employment and random drug testing
- Must have a valid driver s license, good driving record, and proof of auto insurance
- Must be available to work weekends and holidays
- Assess skills and interests of clients
- Provide direct care and support to individuals with developmental and intellectual disabilities
- Document services provided to clients in EVV/OASIS
- Assist individuals with daily living activities, including personal hygiene, meal preparation, medication administration and transportation
- Foster a safe and inclusive environment to promote independence, socialization and community integration
- Implement individualized care plans and document progress
- Collaborate with families, caregivers and interdisciplinary team to ensure high-quality care
- Promote and facilitate skill development and personal growth
- Accompany participants to community activities as needed
- Work as a team with co-workers and supervisors; maintain a respectful, positive attitude
- Complete initial and annual trainings (CPR, First Aid, Tb skin assessment, etc.) as directed by policy
- Attend monthly staff meetings and other trainings as scheduled
- Maintain consistent attendance and adhere to agency policies
- Foster collaborative relationships among leadership, management, and programs
