Overview

Assist residents with activities of daily living, monitor health, safety and well-being, and support the activity program. Provide Hands On support to residents and help maintain a safe environment. Carry out duties as assigned by the Supervisor. Attend monthly meetings and read relevant resident information in staff communication logs, daily progress notes and flow sheets.

• Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility
• Assist with the activity program; conduct activities as directed by the Activity Director or assigned by supervisor
• Monitor health, safety and well-being of the residents
• Document information pertaining to residents’ functional status or condition in appropriate flow sheet binder
• Provide Hands On support/Eye View to residents
• Assist in keeping the environment safe for residents
• Report any changes or concerns regarding residents’ condition to D.O.N. and/or Nurse on staff
• Maintain an accurate record of resident meals intake, output and bathing record
• Do personal laundry for residents
• Carry out other duties as assigned by the Supervisor
• Attends/Participates in monthly meetings as well as reads all pertinent resident information in staff communication log, daily progress notes and flow sheets

Must have current CMT certification
