Overview
The Activities & Care Coordinator is responsible for providing leadership in the planning, organizing, and implementation of life-enhancing activities for residents, and in providing auxiliary direct support and care to residents. The Activities & Care Coordinator collaborates with team members to create person-centered programming for residents and inspire participation.
CompensationHourly paid position beginning at $20 per hour, increasing with relevant experience. A $1.00 weekend shift differential is added when performing direct support, and additional compensation is provided for on-call responsibilities. A lucrative vacation and sick time package, along with employee benefits, are available.
Qualifications- At least 21 years of age
- Degree in a related field or 2 years of experience with adults who have intellectual disabilities
- Good working knowledge of computers and Microsoft Office
- Ability to work independently, take initiative, and be self‑motivated
- Valid driver’s license and automobile insurance
- Good written and verbal communication skills
- Good organizational skills and adherence to deadlines
- Demonstrated understanding and commitment to the philosophical tenets of community‑based services

- Completes supervisory training modules on CDS within 6 months of hire date
- Conducts self-assessment according to the organization’s expectations of professionalism and confidentiality
- Assists and cooperates with family member requests
- Ensures a safe, clean, and comfortable living situation
- Provide central leadership to planning events and outings by collaborating with Program Manager, House Manager, and Development team
- Develop a monthly activities calendar
- Develop annual life enrichment plan and assist with budgeting
- Work with House Managers and Lead DSPs to assess resident interest
- Promote cross‑household experiences
- Promote community involvement
- Develop community resources and contacts in conjunction with the Arts & Wellness program
- Capture photos of activities and provide to the development team
- Track resident participation in all activities
- Create and maintain a token economy to implement in the Arts & Wellness Program (Wellness Bucks/Store)
- Act as Program liaison to Arts + Wellness program
- Provide leadership at resident activities and events, modeling best practices and ensuring resident safety and enjoyment
- Review, monitor and provide leadership and training to all new support staff in the following areas
- Provide mentoring and ongoing support to new employees in their first 90 days of employment
- Assess competency of post‑independent function staff to identify ongoing training needs
- Provide Leadership to DSPs on weekends (alternating with other PA)
- Rounds at every house to assess for need/opportunity
- Give direction/suggestions to staff
- Promote outings and activities
- Ensure the weekly schedule is posted in each home
- Answer Helpline calls on assigned weekend from 10 p.m. Friday until 8 a.m. Monday
- Assist with ensuring effective communication between houses and the office, including but not limited to
- Monitoring that accurate, up‑to‑date staffing schedules are posted in each location
- Participation in assigned email groups (house medical appointments, Helpline, etc.)
- Maintenance and use of shared documents (schedules, Site Visits, etc.)
- Knowledge and use of comprehensive communication tools (e.g., house communication logs, email, meetings, etc.)
- Provide leadership and support to resident medical appointments
