Twelve Stone Health Partners is focused on the medication needs of patients with chronic, complex and rare conditions. We support the transition from acute to post‑acute care environments and the journey from sickness to health. We are currently licensed in 50 states.

We currently have an opening for a Part‑Time Pharmacist‑In‑Charge position in the Murfreesboro area.

Summary

This position will serve as Pharmacist‑In‑Charge of the local branch and is responsible for preparing and administering appropriate pharmaceuticals for patients, either by executing a physician’s order or by addressing the patient’s problems and needs. The role also clinically manages Home Infusion and Specialty medications, assists with the onboarding of new hires, trains staff to manage patient orders efficiently, checks prescriptions filled by pharmacists and pharmacy technicians, maintains professionalism, and ensures pharmacy compliance with federal and state regulations.

Essential Duties & Responsibilities
  • Coordinate with medical professionals to facilitate referrals and deliver pharmaceuticals to patient homes and collaborate with Twelve Stone internal nursing services for safe home nursing administration. Direct involvement in clinical management and assist other clinical staff to assure consistency and accuracy of all prescriptions dispensed.
  • Prepare and dispense appropriate pharmaceuticals to patients, either by executing a physician’s order or by addressing the patient’s problems and needs, including verbal orders over the phone from physicians and other health care providers.
  • Document patient care activity, home infusion nursing services provided, and all communication with patients and/or physicians. Monitor patient progress to collaborate on home infusion goals, assess medical needs, psychosocial needs, and the home environment, and provide appropriate intervention and instruction for patient/caregiver to safely administer care at home.
  • Provide oversight for location, serving as PIC with the Board of Pharmacy and ensuring all rules, regulations, and accreditation standards are compliant.
  • Other duties as assigned.
Requirements

Education: A Bachelor’s or Pharm

D degree from an ACPE‑certified school of Pharmacy required. Graduate level degree preferred. Current, unrestricted Pharmacist license in the state of practice required.

Experience: Minimum two years’ experience serving in a PIC role. Specialty pharmacy and/or 1–2 years in home infusion pharmacy preferred. Must have an active, unrestricted TN Pharmacy license and/or be willing to sit for the MJPE within the first three months of employment. Must be proficient in MS Office, including Word and Excel.
