CHARACTERISTICS OF THE WORK

Under the direction of the Family Health Centers’ (FHC) Chief Pharmacy Officer, provides advanced pharmaceutical care to FHC patients as a key member of an interdisciplinary care team.

Responsible for providing direct patient care, medication therapy management (MTM), and chronic disease management.

Responsible for ensuring compliance with federal, state, and local regulations.

EXAMPLES OF THE WORK
  • Conducts comprehensive medication reviews for patients with chronic illnesses (e.g., diabetes, hypertension, human immunodeficiency virus, asthma).
  • Collaborates with primary care providers to optimize pharmacotherapy and develop care plans.
  • Provides direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other personnel.
  • Provides medication counseling and adherence support directly to patients.
  • Manages medication therapy under collaborative practice agreements, including initiating, adjusting, or discontinuing medications where permitted.
  • Participates in interdisciplinary team huddles and case conferences.
  • Monitors patients for adverse drug reactions, interactions, and efficacy of therapies.
  • Supports the management and compliance of the 340B Drug Pricing Program, including auditing and reporting.
  • Serves as a preceptor to pharmacy students and residents, as applicable.
  • Assists in the development and implementation of clinical protocols and formularies.
  • Maintains accurate and timely documentation in the electronic health record (EHR).
  • Serves as a key participant on FHC’s Quality Improvement (QI) Committee regarding quality improvement initiatives related to medication use and patient outcomes.
  • Ensures pharmacy services comply with the Health Resources and Services Administration (HRSA), Joint Commission, state board of pharmacy, and other relevant bodies.
  • Supports pharmacy operations as needed.
  • Engages in ongoing professional development in accordance with public health standards and licensing requirements.
  • Participates in public health initiatives, emergency preparedness, or disaster response efforts as required.
  • Performs related work as required.
MINIMUM REQUIREMENTS
  • Doctorate degree in Pharmacy.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
  • Responds to patient and Family Health Centers needs via the telephone, internet, fax, computer software and in person.
  • Lifts and carries materials weighing up to 30 lbs., with or without assistance.
  • Spends extended periods of time standing, climbing, and reaching.
SPECIAL REQUIREMENTS
  • Possession of an active license in good standing to practice Pharmacy in the state of Kentucky (License must be clear of any actions that would prevent payment from Medicaid or Medicare).
  • Possession of an immunization administration certification.
  • Possession of a Naloxone certification.
  • Must provide own transportation to work at other locations as needed.
  • Must be available to work extended hours as necessary.
