COMPENSATION:
Salary $260k+ Incentive Bonus + Signing Bonus up to $50k

We need the best primary care providers and clinical leaders who wish to fulfill purpose, personal opportunity, and join our team. Transportation is provided to patients that need it at no charge.

• Compensation:
  Base $250k + Bonus (up to 10% of Base Salary)
• Partnership:
  No Buy‑in
• 18‑20 patients a day
• Hours:
  
  Monday – Friday;
  Clinic Hours 7:30 a.m. – 5:00 p.m. (last patient scheduled at 4:15 p.m.)
• Work/Life Balance
• Small Patient Panel – 450 patients max
• Administrative time included in schedule
• No nights or weekends
• 100% outpatient
• Dedicated Care and support team (own MA/Scheduler/Referral specialist)
• Patient Population:
  Seniors with 3‑5 chronic diseases
• Value‑Based Care
