Your Duties Include
  • Provide welcoming, trauma-informed engagement while undertaking assessments and offering brief interventions
  • Develop collaborative, person-led care plans in coordination with internal and external providers
  • Co-facilitate psychosocial groups, wellbeing workshops, community-based programs and deliver outreach when needed
  • Maintain timely, accurate and professional clinical notes and data entry
