Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Tri-Community Nursing Center. The LPN will provide high-quality direct patient care and support to our nursing staff under the supervision of a Registered Nurse (RN) and physician. The ideal candidate will possess excellent interpersonal skills, attention to detail, and a strong desire to make a positive impact on the lives of our residents.

Responsibilities
  • Provide direct patient care, including administering medications and treatments as prescribed by the healthcare provider.
  • Monitor and record vital signs, patient health status, and changes in condition.
  • Assist patients with daily living activities, including bathing, dressing, and meals.
  • Maintain accurate and up-to-date patient records and documentation.
  • Collaborate with RNs, physicians, and other healthcare professionals to develop and implement patient care plans.
  • Educate patients and their families about health conditions, treatment options, and preventive care.
  • Ensure a safe and clean environment for patients, adhering to infection control protocols.
  • Respond to patient calls and address their needs and concerns promptly.
  • Assist in the orientation and training of new nursing staff or student nurses as needed.
  • Participate in team meetings and contribute to quality improvement initiatives.
Qualifications
  • Valid and active LPN license in Louisiana.
  • Completion of an accredited practical nursing program.
  • Previous experience in a healthcare setting preferred, but not required.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate, patient-centered approach to care.
  • Excellent organizational and time management skills.
Physical Requirements
  • Ability to lift, move, and assist patients as needed.
  • Ability to stand, walk, and perform physical tasks for extended periods.
  • Capacity to manage the demands of a fast-paced healthcare environment.
