This position handles multidisciplinary communications and facilitation of care throughout a patient’s hospitalization. The role reviews the appropriate utilization of resources, balances them with the patient’s right to self‑determination, and confirms a clear, specific level of care order and other documentation in the medical record. The nurse coordinates care with the patient, caregiver, multidisciplinary team, post‑acute and third‑party payers, and collaborates with the team to develop and implement discharge plans based on individualized needs.
Education- Registered nurse diploma or equivalent (required)
- Bachelor’s degree in nursing (preferred)
- None (required)
- Three years of hospital‑based experience in discharge planning, case management, or utilization review (preferred)
- Current registered nurse license in state of practice (required)
- Basic Life Support (BLS) from the American Heart Association (required)
- Certification in Case Management (CCM) or Accredited Case Manager (ACM) (preferred)
- Proficiency in using computers, software, and web‑based applications
- Effective verbal and written communication skills; ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
- Strong interpersonal skills
- Ability to work a flexible schedule, including 24/7, weekends, holidays, and on‑call availability
- Performs pre‑admission, admission, and continued stay reviews as directed per CMS guidelines, commercial payer guidelines, and/or hospital policy
- Supports the values of service excellence, teamwork, and integrity; maintains licensure standards and pursues personal/professional development
- Maintains knowledge of conditions of participation related to utilization review and discharge planning
- Maintains open communication with all appropriate parties while functioning as a liaison between ED physicians, hospitalists, attending physicians, and physician advisors
- Leads oversight and management of functional and financial outcomes, including coordination and implementation of the discharge plan during acute‑care hospitalization
- Adapts behavior to the specific patient population, respecting privacy, introducing oneself appropriately, explaining services and procedures, requesting permissions, and tailoring communication style
- Performs other related duties as required
Medium work:
Exerting 20–50 pounds occasionally or 10–25 pounds frequently, and/or greater than negligible up to 10 pounds constantly to move objects. Exposure to blood, body fluid, and tissue; occupational risk for hazardous medications or waste.
