Position: Pharmacist PRN Premium
Location: Napoleonville

Job Description

Provides proper and adequate pharmaceutical service, including preparation and dispensation of all drugs. The Staff Pharmacist ensures proper storage, packaging, stability, and disposition of pharmaceuticals.

Responsibilities

Patient Prescriptions and Profiles:

  • Reviews and interprets patient prescriptions and profiles in order to prevent overdoses, incompatibilities, and toxicity and to ensure the provision of high quality pharmacy services and patient care.
  • Fills package forms with measured amounts of medication according to prescription.
  • Types information for identification and use of medication and attaches label to container following the general dispensing format and in an effort to ensure proper medication intake by patients.
  • Efficiently dispenses additives for IV and irrigating solutions, and mixes IV additive solutions using aseptic techniques, and the laminar air flow hood.
  • Able to effectively work with Automated Dispensing Machines and other technology associated with barcode labeling, packaging and storage of medications.
Quality and Performance Improvement
  • Maintains records on filled prescriptions, documents required information on the patient receipt form, and maintains inventory records on narcotics and other forms of drugs as required by law.
  • Inspects pharmacy inventory to ensure that outdated drugs are removed from stock and replaced monthly in order to control hospital/retail pharmacy costs and ensure patient safety.
  • Treats all employees with compassion, understanding, and respect, and strives to enhance performance through cooperative effort and action.
  • Participates in educational programs, in-service meetings, and training sessions in an effort to share his/her own expertise with others and further the quality of education and personal growth provided to new personnel, volunteers, and interning students.
  • Observes and adheres to all departmental and hospital policies and procedures, and follows all safety, quality assurance, and infection control standards.
  • Promotes the quality and efficiency of his/her own performance through participation in staff educational programs, approved continuing education courses, and specialized skill training programs.
Collaboration and Partnership
  • Consults with physicians and nursing staff/customers concerning the availability, side effects, and possible incompatibilities of new as well as currently used drugs.
  • Confers with physicians regarding prescription questions or irregularities in an effort to ensure patient safety.
  • IV Admixture Services
  • Prepares, procures, packages, stores as well as distributes sterile products & supplies using aseptic technique for adult as well as pediatric patients within established procedures & legal requirements.
  • These therapies include, but are not limited to: antibiotics, small & large volume parenteral, pain medications, and sterile ophthalmic preparations.
  • Document responsibility by signing (electronically when available) preparations, orders, and other forms.
  • Provides drug information regarding drug compatibilities to other health care providers as well as assists in managing complex patients (fluid restrictions, latex sensitivities, etc).
  • Specifically reviews all physician orders for pediatric orders ensuring that the dosing, administration and pharmacist order entry are correct.
  • Documentation of all calculations and approvals must be readily retrievable departmental & procedure.
  • Orders & maintains drug, supply, & equipment inventories for the delivery of the admixture service.
  • Maintains work area to ensure storage & work areas are clean & organized for a safe work environment.
  • Resolves drug, supply & equipment problems as needed.
  • Demonstrates professional competency in accordance with current sterile compounding standards & participates in ACPE accredited continuing education programs.
  • Monitors department staff for errors, adherence to the established department procedures, and compliance with standard order entry practices.
  • Documents all quality problems, clinical interventions, & medication safety events using the approved methods.
  • (Medkeeper, Quantros, Simplifi 797) Maintains proper records related to medication preparation, controlled prescription record keeping & other departmental records.
  • Demonstrates proficiency in operating automated systems for verification, patient profile review or retrieval of clinical data.
  • Ensure that waste minimized and that all occurrences are electronically documented by nurses.
Other Duties As Assigned
  • Performs other duties as assigned or requested in an Emergency Department setting.
Qualifications

Education:

Bachelor's Degree;
Pharm.

D.

Licensure:
Licensed as a Registered Pharmacist in Louisiana.
