Overview
Be among the first 25 applicants. The Social Worker LCSW provides clinical social work services for patients and their families, conducting psychosocial assessments and discharge planning for patients with complex psychosocial and medical problems.
ResponsibilitiesConduct intensive psychosocial assessments through interviews, family collaboration, and medical record review. Identify coping skills, cognitive functioning, support systems, and barriers to treatment. Communicate findings and create intervention plans with the interdisciplinary team. Provide short‑term supportive counseling for temporary or situational issues. Perform assessments for suspected elder, child, sexual, or domestic abuse or neglect and comply with required reporting. Utilize crisis intervention skills to assist victims/families of abuse.
Act as a primary team member in discharge planning, identifying social and financial factors and barriers. Facilitate family meetings and resolve care decision conflicts. Maintain knowledge of community resources and establish relationships with regional agencies and admissions staff.
Licensed Clinical Social Worker (LCSW) from the Louisiana State Board of Social Work Examiners. Bilingual in Spanish strongly preferred.

Work ShiftDays (United States of America). Monday – Friday, Day shift.
