Job Summary:

Odyssey House Louisiana, Inc. is seeking a full-time Outreach Coordinator for the Prevention Program. The schedule is Monday-Friday 8am-5pm; weekends and after-hour events will be required. This role is a full-time grant funded position by the Hepatitis C Elimination Initiative Pilot. The Outreach Coordinator is responsible for developing and implementing outreach strategies to raise awareness of Hepatitis C, which includes prevention, testing, and treatment services.

The position will serve a diverse and vulnerable population and require coordination with community members and stakeholders to connect individuals to screening, care, and supportive services. The role will also involve accurate tracking, data entry, and ensuring all outreach activities align with agency values and protect client privacy.

Duties & Responsibilities
  • Maintain comprehensive documentation of all activities for data collection
  • Refer individuals to the Agency for primary medical care, behavioral healthcare, infectious disease screening and wraparound services
  • Assist in development and conduct community trainings on infectious diseases and public health information
  • Assist in development and conduct community harm reduction events and efforts
  • Refer individuals to the OHL Community Health Center for primary medical care and treatment services
  • Refer and follow up on the receipt of services
  • Coordinate, consult and collaborate with service providers along with the Multi-disciplinary team
  • Provide Medicaid Enrollment to populations
  • Complete all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure
  • Treat clients, community members, and other staff with dignity and respect; organize workload to complete responsibilities in an appropriate and timely manner
  • Maintain an up-to-date work calendar
  • Maintain comprehensive documentation of all activities for data collection
  • Present potential partnerships to the Program Supervisor for approval
  • Responsible for administrative tasks as directed by the Program Supervisor
  • Conduct community outreach as directed by the Program Supervisor
  • Assist with special projects as directed by the Program Supervisor

Minimum Qualifications
  • At least 1 year of outreach and/or education to various populations experience
  • Excellent communication and interpersonal skills
  • Familiarity with the community served by the Agency
  • Proficient in MS Office Suite
Preferred Qualifications
  • Master of Public Health or equivalent
  • One year experience providing health education to various populations
  • Skilled and experienced in delivery of health education topics to diverse populations
  • Knowledge of various healthcare issues affecting the community served by the Agency
  • Bilingual – English/Spanish, English/Vietnamese
  • Certified Medicaid enrollment preparer
  • EHR experience
  • CTR certified
