Job Summary
Provides skilled physical therapy services to patients with impairments or disabilities. Plans, implements, and evaluates therapy programs to improve mobility, reduce pain, and promote independence.
Responsibilities- Perform evaluations and develop individualized treatment plans.
- Implement therapeutic exercise, manual therapy, and modalities.
- Educate patients and caregivers on treatment and home programs.
- Document progress in compliance with regulations.
- Supervise PTAs and aides per state law.
- Collaborate with interdisciplinary team members.
Physical Therapist (PT) Reports To Director of Rehabilitation
- Essential Duties & Responsibilities
- Perform evaluations and develop individualized treatment plans.
- Implement therapeutic exercise, manual therapy, and modalities.
- Educate patients and caregivers on treatment and home programs.
- Document progress in compliance with regulations.
- Supervise PTAs and aides per state law.
- Collaborate with interdisciplinary team members.
- Doctorate, Master’s, or Bachelor’s Degree in Physical Therapy.
- Current state license to practice Physical Therapy in good standing.
- Experience in long-term care or rehabilitation preferred.
- Knowledge of PT principles and practices.
- Strong communication and documentation skills.
- Effective time management.
- Ability to lift/push/pull up to 100 lbs with assistance.
- Frequent standing, walking, bending, kneeling, and reaching.
- Work in healthcare and long-term care facilities.

Qualifications:
A Doctorate, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program is required. A current and valid state license to practice Physical Therapy is required and must be maintained in good standing. Previous experience in long-term care or rehabilitation settings is preferred.
