The Clinical Pharmacist works with physicians, care managers, and other clinical staff as part of a medical care team focused on patient outcomes and cost containment. Responsibilities include therapeutic assessments, disease management, and coordinating with hospital or specialty pharmacy operations.
General Duties- Direct and initiate development, implementation, and maintenance of clinical pharmacy services.
- Provide clinical consultation to the Medical Staff and other providers, recommend cost‑effective alternatives.
- Communicate and document all medication‑order changes, pharmacokinetic monitoring, and adverse drug reactions with prescribers.
- Collaborate with all health‑care providers to resolve concerns and questions.
- Develop and disseminate educational materials and in‑services for physicians, nursing, pharmacists, and other professionals.
- Facilitate treatment guidelines, protocols, formulary changes, and medication‑use evaluations.
- Participate on clinical committees (pharmacy & therapeutics, etc.) and maintain knowledge of JCAHO/CMS standards and State Board of Pharmacy requirements.
- Ensure compliance with specialty‑pharmacy accreditation standards (ACHC, URAC) and perform audit, quality‑improvement projects.
- Lead prior‑authorization and appeal processes and maintain knowledge of payer policies.
- Maintain 340B capture and compliance with patient definition and coverage requirements.
- Pharm.
D. with at least 2 years of inpatient or outpatient hospital/clinical pharmacy experience, or completion of a Pharmacy Residency. - For specialty pharmacy roles, preference for Pharm.
D. with 2 years of retail or specialty pharmacy experience, or residency completion. - Licensed or registered to practice pharmacy in Louisiana.
- Board certification in an applicable specialty area preferred.
