This job assumes responsibility for the medication management of assigned patients; monitors and evaluates medication therapy for appropriateness based on the patient's health status, institutional criteria/guidelines, and cost‑efficient measures, including medication reconciliation, monitoring, and patient education. You will remain integrally involved in the education of pharmacy and medical residents, provide education to health care providers as needed, and offer pharmacy expert content for order sets, benefit design, formularies, and other pharmacy‑related documents.

• Provide pharmacokinetic consultation, obtain and use appropriate laboratory data to evaluate drug selection and dosing, and communicate findings to health care practitioners.
• Evaluate patient response to drug therapy.
• Document interventions and conduct outcome studies/evaluations related to clinical activities performed.
• Assist with development of pathways, best practices, and guidelines for assigned population.
• Educate staff, students, patients, health care providers and other health care professionals; submit for publication once every two years.
• Mentor residents, students, and new staff to meet departmental goals.
• Responsible for overall patient care beyond the hospital visit, through all transitions of care.
• Volunteer to perform tasks in technical and non‑technical areas during staffing shortages caused by inclement weather, power outages, computer downtime or other situations.
• Adapt behavior to the specific patient population, respecting privacy, explaining services, requesting permissions, and adjusting communication style.
• Perform other related duties as required.

• Education:
  
  Bachelor of Science in Pharmacy; completion of PGY1 Pharmacy Practice Residency (3 years of related specialty experience accepted).
  Preferred: Doctor of Pharmacy (Pharm
  
  D) and completion of PGY2 Pharmacy Residency.

• Certifications:
  
  Current Pharmacy license in the state of practice.
• Knowledge, Skills, and Abilities:
  Proficient computer skills for data entry and retrieval; effective verbal and written communication; strong leadership/teaching skills.

Medium Work – may require exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently or greater than negligible up to 10 pounds constantly. Physical demands exceed those for sedentary work. Must be able to stand for prolonged periods of time, travel within and between facilities, and handle exposure to blood, body fluids, tissue, communicable diseases, hazardous medications and waste.
