This role assumes responsibility for medication management of assigned patients; monitors and evaluates medication therapy for appropriateness based on the patient's health status, institutional criteria and guidelines, and cost‐effective measures including medication reconciliation, monitoring, and patient education. The pharmacist remains integrally involved in the education of pharmacy and medical residents, provides education to health care providers as needed, and supplies pharmacy expert content for order sets, benefit design, formulary, and other pharmacy‐related documents.

The Pharmacy Department comprises inpatient and outpatient pharmacies providing administrative, clinical, and distributive services. Pharmaceutical specialties include Transplant, Cardiology, Geriatrics, Anticoagulation, Critical Care, Infectious Diseases, Pediatrics, Internal Medicine, and Oncology. Ochsner Ambulatory Pharmacy Services includes Anticoagulation Clinic, Digital Medicine, Population Management, and Pharmacy Patient Assistance.

• Graduate of an accredited School/College of Pharmacy
• Completion of a PGY‑1 Pharmacy Residency required; completion of a PGY‑2 Pediatric Specialty Residency or Fellowship preferred
• Licensed to practice pharmacy in the State of Louisiana or eligible for licensure
• Current pharmacy license in the state of practice

• Computer skills and dexterity for data entry and retrieval of patient information; proficiency with Windows‑style applications and keyboard.
• Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
• Strong leadership and teaching skills.

• Provide pharmacokinetic consultation, obtain and use appropriate laboratory data to evaluate drug selection and dosing, and communicate findings to health care practitioners.
• Evaluate patient response to drug therapy.
• Document interventions and conduct outcome studies or evaluations of clinical activities.
• Assist with the development of pathways, best practices, and guidelines for the assigned population.
• Educate staff, students, patients, health care providers, and other professionals; submit findings for publication periodically.
• Mentor residents, students, and new staff to meet departmental goals.
• Ensure overall patient care beyond the hospital visit, through all transitions of care.
• Volunteer to complete tasks during staffing shortages caused by inclement weather, power outages, computer downtime, or other situations.
• Adapt to the specific patient population, respecting privacy, introducing procedures appropriately, and communicating effectively.
• Perform other related duties as required.
